This is an example from the District on how to obtain reimbursement for hearing aids if you are covered by PERS Choice Medicare Supplement, PERSCare Medicare Supplement or, starting in 2022, PERS Platinum Supplement to Medicare. The example uses Costco, but the items that you need to include are the same for all vendors. Please note that you must send the request for reimbursement within 90 days of the date of your bill or you lose the chance to be reimbursed the allowed amount.
